myChart Proxy Access

Proxy access allows

parents, legal guardians and caregivers to log into their personal myChart accounts, and then connect to medical information for the patient they represent. Available access is based on age of the patient and proper authorisation.

With Partial Access, proxies can:

See virtually of what patient can view, excluding information related to sensitive topics and communicate with the doctor’south office.
Proxies do not have the ability to:

Asking prescription refills or view appointments.

With Full Access

proxies can enjoy the same features and benefits bachelor to the patient including access test results, Communicate with the doctor’south role, manage appointments and request prescription refills.

Patients 11 & under

Partial proxy access

does not employ to this age group.

Full proxy access

is available to parents and legal guardians

Patients 12 – 17

By default

proxy access is limited to help protect the privacy of our patients. Admission changes begin on the patient’s 12th birthday.

Full access is bachelor

With the agreement of the patient’south trusted care provider and patient authorization, full access is available. Proxy access is reassessed annually and ends on the patient’s eighteen birthday.

Teen Proxy
 Allows a parent or guardian to take
limited
access to myChart business relationship of pediatric patient
between the ages of 12-17.		 Admission is obtained by completing the
myChart Proxy Class
and providing it to the patient’south Care Provider.		 Access is automatically terminated on the kid’s 18th altogether.
Teen Proxy – Enhanced
 Allows a parent or guardian to take full access to myChart account of pediatric patientbetween the ages of 12-17. Admission is obtained after the teen patient
consents to the parent/guardian having total access
and past the teen and parent/guardian completing themyChart Proxy Form
and providing information technology to the patient’s Care		 Access is automatically terminated on the child’south 18th birthday.
Developed Proxy
 Allows an individual to accept
fullaccess to the myChart account of an adult
eighteen years of age or older.		 Access is obtained by the patient
inviting the proxy
online from inside his/her myChart account or in clinic where both complete the
myChart Proxy Course
and providing it to the patient’s Care Provider.		 Access is terminated when the patient makes a written or online request to stop access, an expiration date specified is reached, or the patient revokes access from within their myChart account
