All unvaccinated patients are required to wearable a mask during their appointment.

Our New Patient Information Course:
This fillable course will be sent directly to our role and salve time at check-in.
Fill out 24 hours in advance
of your appointment.
Nosotros will also verify this information
at the time of your appointment.

Photo ID:
Federal Law requires proof of identity to submit to an insurance carrier. If unable to
provide a photo ID, Patient will exist required to pay for services up-front.

Social Security Number:
We require the social security number of the responsible party to
institute an account that carries any balance. If you do non wish to supply your social security
number, we are happy to provide your medical intendance, however, we will require you to pay for all
service in full prior to being seen

Current information: Bring all current insurance cards and provide current demographic
information including address, phone numbers and employer.

Things to Know

Surgery: If you are being referred to our office for any kind of surgery your first appointment with our office will not exist surgery. Nosotros will perform an evaluation and our surgeons will talk to yous about all surgical options and schedule surgery at this time. You will receive a separate packet of information for your surgery twenty-four hour period.
*This may exist different for iLASIK or PRK patients.

Prior Authorizations:
Some insurance plans crave a prior say-so for services by a
specialist. Review your policy to run into if there is such a requirement. Our office will be happy to
assistance you in obtaining prior authorizations or approvals when needed but remember it is your
responsibility to know the requirements of your policy. Failure to obtain prior approval or prior
authorization could result in a service being non-covered, which will effect in higher out of pocket
costs for you.

Medical Facility: We are a medical facility and do not participate with any vision plans.
